Open Position Information

  • Position Requisition – To open a position, a signed Position Requisition (P.R.) must be submitted to your Service Team. You may find an online requisition here (Use your Campus Information System unid/password for access).

  • Under- Utilized Positions – Please contact your Human Resource Service Team for more information.

  • Veterans Preference – Managers should interview Veterans Preference eligible applicants, who meet minimum qualifications, subject to the Veterans Preference Procedure.  Veterans Preference Procedure is available at the following link,  http://www.hr.utah.edu/forms/lib/Addendum.pdf .  For further information, contact your Human Resources Service Team.

  • Selection Criteria – Managers should prepare a summary of the selection criteria used to evaluate job applicants and retain a summary with hiring materials in department files. Selection criteria examples can be found here.

  • Reduction in Force (RIF) Preference – Managers should interview all applicants referred by Human Resources as eligible for RIF preference. For further information, contact your Human Resource Service Team.

  • Advertisement for Staff Employment – To assure compliance with federal regulations, any advertisement for staff employment is to be approved by Human Resources prior to placement.

  • References – The University strongly encourages managers complete reference checks on both internal and external applicants. A sample reference check form can be found here.

  • Drug Testing - All new hires and transfers into patient sensitive positions will be subject to a pre-employment drug screening within 48 hours of receiving a job offer. To register a potential new hire for the pre-employment drug screen to go: https://www.hr.utah.edu/www/screen/.

  • Offer Letter (Recommended) – After a job offer has been made and accepted, an offer letter may be completed and given to the new hire. If an offer letter is completed, one copy should be sent to Human Resources with the new hire forms. You may find the letter here.

  • Non-Selection Letter (Strongly Recommended) - A non-selection letter should be sent to all applicants who were interviewed but not selected for the position. You may find the non-selection letter here.

  • Applicant Flow Record – Upon closing your position, you will receive a computer-generated applicant flow record that indicates all applicants who applied for your position. Please mark your reasons for selection or non-selection based on the job requirements. If an applicant was not interviewed please leave the field blank. Remember that these forms become part of the University’s permanent record for review should the selection ever be challenged. A blank applicant flow record can be obtained from our web site here.

  • Personnel Action Notification (PAN) – A PAN form should be completed for all new hire employees. You may find the blank PAN form here (When prompted, enter the job code in the box to pull up the appropriate PAN form).

  • 2007 PAN Form Submission Calendar - Click here to see PAN Form due dates for 2007.

  • New Employee Orientation – Orientation is provided for Campus, Faculty and Hospital employees. Please click here to determine which orientation is most appropriate for you or your new employee(s).

  • I-9 – All new employees must provide appropriate documentation to verify identity and employment eligibility. For instructions on completing the I-9 click here, and for the I-9 form itself, click here. Any additional questions should be directed to your Service Team.

  • Criminal Background Check– Employment offers are contingent upon completion of criminal background check. You may find a background check release form here.

  • Professional Licensure – All licenses/certifications are due at the time of hire and must be submitted with new hire forms. Hospital employees must also obtain Basic Life Support (BLS) card within one month of hire.

  • Immunizations - (Hospital Only) All new Hospital employees must provide immunization records prior to employment. You may find the requirements and forms at: http://www.hr.utah.edu/hsc/imm/.

  • W-4 – All employees must complete a W-4 for tax purposes. For a copy of the W-4 please visit our web site at: http://www.payroll.utah.edu/downloads/W4Form2004.pdf.

  • Direct Deposit – All new hires are required to have their bi-monthly checks direct deposited. Wells Fargo Bank will allow new employees to open a new account at no charge. For the direct deposit setup information please visit the direct deposit information page.
